You should request for a replacement card if any of the below apply:
a. You have misplaced, lost, or damaged your card.
b. Your card has been stolen.
c. You have changed your name.
d. You have renewed your Visa, and your initial card was issued till the expiry of your old Visa instead of
the full 2/4 years (MSIC only).

1. Raise a ticket with OneStop helpdesk.

2. Complete a statutory declaration within 7 days of becoming aware of your need to replace your card.

3. If your card has been stolen, make a report of the theft to the police and supply a copy of a police report or other information issued by the police regarding the theft to OneStop helpdesk within 7 days of becoming aware of the theft.

4. To apply for a replacement card, login to your account using your credentials and click 'Replace card' under the Home page.

5. Read the details and click the 'Apply now’ button.

6. Select a reason for replacing your card from the drop-down menu, nominate a post office for card collection and click the 'Submit request' button.

7. Attach your completed statutory declaration and supporting documents and click the ‘Submit request’ button.
